Hmm I thought the commands were sent in the order they are shown in the summary - TV on, initial input setting, AVR on, AVR input setting, additional device on (If needed), and THEN the repeat of the TV input select.
The observed behavior is that the devices do start up in that order, and the TV input is set last. So either the remote sends it as I describe above
- OR -
It sends TV on, the all the other devices, waits for the power on delay and the sends the input command twice.
What its not doing is sending TV on, wait power on delay, set input (twice), AVR on etc.
Is there something else I may be missing?

Two points here - if the problem is only when starting from all-off then the Power-On Delay is very much involved, and how do you turn off only the TV - is that other than by the Harmony Off button?
My power on delay is set at the default of 15 seconds, and I only turn it off with my Harmony One.

Ahh - thanks for that tinsight - the setup implies series. In that case it would appear I don't need the extra input command.

That's right nmreuben - the list shown at Review Settings doesn't make the sequence entirely clear.

I seem to have the same problem whith my Samsung DLP HL-5678R
I can find settings for Power on delay and Input delay but not "Inter-Device". I have gone as high as 25 seconds on the power up delay with no improvement. It almost always powers up on whatever input it was powered down on. Once the activity is running, inputs change correctly. I only see the problem on power up. The help button works to correct, but I would rather not do it every time. How do I change this "Inter-Device Delay"?

With your Samsung, be sure the Anynet is turned off, The Inter-device delay is found in the same location as the other delays. At Devices, Your TV, Settings, Adjust Delays. The Inter-device delay would not come into play at power up it your TV is not going to the correct input, this is where the Power On delay for the TV comes into play. The Inter-Device delay would come into play if when changing Activities the TV does not go to the correct input.
Here are the delay settings for my Samsung TV
Power On Delay 13000ms
Inter-Key Delay 0
Input Delay 1000ms
Inter-Device Delay 1000ms
