Squeezbox 7.5.1 not working

I recently installed Squeezbox v7.5.1 and can't get it to run. I have a Windows Vista machine. Checking the application error log, I get message:
Carp::Clan::__ANON__(): DBI Connection failed: DBI connect('hostname=127.0.0.1;port=9092;database=sli
The interface looks fine, but nothing works: I can't play any songs, I can't change the volume, etc.
I've tried starting Squeezbox in safe mode, but it acts the same. When I stop and start the server from the Squeezebox Control Panel, I get the msg 'C:\PROGRA~1\SQUEEZ~2\server\Bin\MSWIN3~1\mysqld.e
Version: '5.0.22-community-nt' socket: '' port: 9092 MySQL Community Edition (GPL)' in the application event viewer.
I've never had a problem with previous versions of Squeezebox on this machine.
Any ideas?

what happens when you go into the control panel (right click on icon in tray and select control panel). On the diagnostics tab there is information that is testing your port connections, etc. (9000, 3483). Do these all say OK?

Also, what player are you using? I assume you've done this, but you do have to select the player within the SbS web interface before you can use the web interface to play a song, change volume, etc.

Diagnostics all say OK, but there's no player found. Likewise, on the player , when I try to connect to the server it can't connect.
I downloaded the 7.5.1 software and reinstalled, but it still acts the same.

I have just the basic Squeezbox. I've been using it (and it's previous incarnation) for about 6 years now. Connected via Ethernet. I've rebooted the modem, the player, the computer, re-installed the software. I noticed one of the error msgs mentionned being unable to connect to MySQL. When I look in Services, it shows SqueezeboxMySQL startup=automatic, but the service wasn't running. When I tried to start it manually, it failed with error msg 1067: process terminated unexpectedly.
Well, I just went back to the player to try to connect again. Still failed, but then I changed the networking to wireless (but didn't complete the action because I didn't have the WPA key with me), then back to ethernet, and it connected! No idea what was going on, but I have music again!!

I am experiencing the same problem with my Duet. Although the player name is displayed top right on the browser window, the pull down shows "Your player was not found ..."
When I login to mysqueezebox.com, the player appears there but is not connected.
With the Duet remote, when I attempt to connect to my music library, which the remote is aware of, I get a "Problem Connecting" error message.
I attempted to download the Duet user manual http://www.logitech.com/images/support/14900.1.0.p
Yesterday, I reset "Factory Defaults" via the remote control (and deleted it from the Player list on mysqueezebox.com) and reconfigured the Duet. It came up fine and started working again, including both playing off my library and streaming from radiotime.
This morning the system is back to the "Your player was not found ... " state. It has reappeared in the mysqueezebox.com list, but is not connected there either?
Any suggestions? Does anybody have a pdf of the actual Duet user guide where the LED colors (and flashing states) on the front of the Duet are explained?

I reset factory defaults again, connected the remote via the duet wifi (direct, not via my wifi router). Logged into mysqueezebox.com account; all is good, can stream radio, can remote control the player.
Now I'll try connecting to music library on computer:
now the browser top right pull down sees both "Midvale_Duet" and mysqueezebox.com
When I select mysqueezebox.com the browser connects to the web site (same window so squeezebox server window disappears).
Press back; select "Midvale_Duet", player control switches over to browser; (top right pull down now only shows Midvale_Duet; mysqueezebox.com entry is gone).
Stream a couple radio stations out of "My Favorites"; good.
Select an album and press play: sound stops; first track is listed in now playing panel of player window (right hand side of browser); queued track list (bottom of player panel) continues to show the last radio station played.
Attempt to bring up another album - press play; cursor 'spins'; browser window closes; squeezebox server crashes.
From remote; attempt to connect player back to mysqueezebox.com ... "Problem Connecting" error message comes up
Restart squeezebox server on PC: .. won't restart .. killed processes in task manager; still will not start. (have to restart laptop) .. will be back.
Rebooted laptop; restarted squeezebox server; comes up in state with player connected to mysqueezebox.com BUT remote control attempt to play something yields "Problem Connecting" error message.
Login to mysqueezebox.com via browser; player is actually connected; using Remote Control (browser window via mysqueezebox.com), am able to play radio station ... now remote is synch'd with player with "Now Playing" showing the correct station. Can now change stations at will with handheld remote.
now try squeezebox server on PC again: via top right pull down, select "Midvale_Duet", respond yes to pop-up asking whether to switch from mysqueezebox . Squeezebox server hangs .. cursor is in spinning circle mode ... fubar
Hypothesis: Something has changed within the Duet - mysqueezebox.com control software; BUT was not tested for backward compatibility with the squeezebox server software with the result being Squeezebox 7.5.1 software is now seriously incompatible with mysqueezebox.com / Duet software.
Result: local music library is useless; workaround is to use mysqueezebox.com only.

I don't have any suggestions (although it is not clear if you tried: 1. turn off router, computer running SbS, unplug player. Then turn on router, let fully start, reboot computer let SbS fully start, then plug in player. Sometimes IP addresses, DNS server, etc. etc. gets messed up and letting everything get reassigned sorts things out. Sounds like the player can see the internet, but can't see the SbS properly. Anyhow, worth a try.
But regarding your hypothesis, I don't think that is a possibility. I used 7.5.1 and now 7.5.2 with a duet (and with boom, transporter, touch, SB3) and it worked fine (this was with both a XP machine and separately with a WIN 7 (64) machine. And I currently use 7.5.1 on a vortexbox appliance. But bottom line, there is no compatibility problem between 7.5.1 and the duet and/or mysqueezebox.com.

I queued on your reference to 7.5.2 and installed it. During the install it complained that it could not install faad.exe because it was in use; I killed the faad process in Task Manager and the install ran to completion.
SBS started up fine and, success, it detected my player and I could switch it from mysqueezebox.com to SBS. I then selected a live365 station and it played fine. Then ..
selected an album from my library; fan speeds up - SBS hangs - Task Manager shows CPU at 25% - process consuming the 25% is faad.exe
I'll shut down and restart.

odd issue. haven't seen that reported before (the faad.exe issue). I might uninstall SbS completely, reboot computer, then install 7.5.2 again. What is your OS and what type of files are you trying to play (mp3, aac, flac, apple lossless?).
