Logitech Pro 9000 wont stay connected to computer

I am using the Logitech Quickcam Pro 9000.
M/N V-UBM46
P/N 861464-0000
PID LZ731BR
Windows XP Service Pack 3.
I was able to successfully use my webcam for 4 months. Recently the camera will not stay connected to the computer. I keep getting the error message that "Hardware attached to the USB port has malfunctioned". I uninstall the Quickcam software and the Quickcam driver then reinstall and the same error occurs. I am running 11.8 version of the logitech driver. However, if I install the webcam using the generic Windows webcam driver there are no problems with the camera aside from the reduced functionality that the generic driver provides.
Please help me get the camera working again with the Logitech driver.

Please try reinstalling QuickCam "clean".

The camera stays connected for 4-5 minutes and then it disconnects and the only way to get it to reconnect is by unplugging the USB connection and then replugging it in.
