Quickcam Blurry Video in Vista

The video is so blurry it looks like I am using one of the effects. How do I correct the blurry image? Typical video in image below.
I have a Quickcam for Notebooks.
1) 860-000010 USB
2) Quickcam Version 11.5.0.1169
3) Windows Vista Biz. SP2
4) The issue occurs in the Quickcam app. and inside Skype.

For the QuickCam® for Notebooks (Washington, USB\VID_046D&PID_08DD):
- Focus Type = Manual Focus Ring located in front of camera (Adjustable)
